I see it as more of testament of a man conceding to his vices. It is actually a pretty sad song if you really pay attention to the lyrics. A bit of trivia: Lou stopped playing this song live for a long time after people told him it inspired them to try heroin.

I don’t think it has to be about drugs, or addiction in general, although it’s a very vivid portrait. Personally I put this on whenever I’m feeling overwhelmed and need to escape all the noise and weight of whatever’s going on. One of the best songs I’ve heard no doubt

i think it's cathartic, no matter if you ever put a needle in your arm or not it talks about dreams, about feeling overwhelmed, and choosing destruction as a way of coping with life
